Important requirements for NRE holders to fillup (for Bajaj allianz policy)

Data sheet must be fillup by computer and printout for get signature of policy holder.

 

Whatever needs?
1. Fillup Data sheet.
2. NRI Questionare form to be fillup by policy holder
3. illustration sheet printout and get signature from website i noted below.
Click this link http://www.bajajallianzlife.co.in/ and see the left side "Premium Calculator" click that. From "unit linked products" select "New Unit Gain RP" and click near the "show packages" button. Now fillup the following details carefully.
  • Name: (policy holder name with initial by capital letter)
  • EntryAgeLA (policy holder age enter)
  • select gender > Male or Female
  • Product details coloum: premium > enter 12000, policy term > 20, Multiplier > 34, Mode> yearly.
  • Don't select anything from "Covers Desc" coloum
  • From "Fund details" see > Equity Index Fund II , select 100.
  • Now click the button of "Show Benefit Illustration"
  • next page will appear now, go down see "While printing" how to make print the whole pages by landscape.
  • Click button of "Print" -3pages will printout and get signature from the NRE policy holder.
Get one colour photo with self attested signature of BLACK INK.
Get passport copies includes visa stamping page and depature, arrival stamping pages.
Get self attestation signature from all pages of passport copies (down side)
If address same as passport, it's ok. Otherwise get Rasan card copy or Telephone bill copy.
